Has anyone developed a way of capturing the information required for the Family Orginin Questionnaire as part of ANC screening?

You could create a rule panel.
You could use picklists for the female and baby's father origins with the choices that match the form. The choices for "other" origins could then have an action attached to trigger another rule which would be a pop up box to type into.
The EDD and Gestation at time of test would be mandatory. I would use a date entry for EDD and then either a numerical entry or picklist for gestation.
The answers will print on the form, however it will depend upon how you are interfaced to your LIS whether or not you are easily able to transfer the information into the LIS. We use DES and therefore anything like this requires Sunquest to do additional work.

I don't see the need for FOQ data to be imported in to the LIMs - Pathology is a diagnostic service not a data repository/informatics service.
I'd be happy to set up the questions and answer choices in ICE but would prefer ICE to generate reports with the collated data for the AnteNatal services so it does not impact on our system.
Can this be done?
Has anybody done it already?
