Happy New Year.
·          Could I please ask you to reply to me within this forum with suggestions about what you would like Anglia ICE to do for you within radiology?
·          What future developments would you like to see?
·          Could I ask that you respond with your top 3?
·          I have a few starter points to get your imagination running
o         Intuitive rules, that search a patient record and provide the answer, if it does not have an answer then request automatically to provide the answer.
o         Symptomatic requesting: - Where the requestor selects the patient’s symptoms and all tests including Pathology are requested.
o         Discharge letters for radiologists : - We currently undertake a lot of therapeutic procedures within radiology
o         If a patient is registered as having a positive result for ‘C Diff’ or ‘MRSA’ then all subsequent requests include this statement until the patient is given all clear.
o         If the patient has long term conditions, then hold this information and include it requests: - Diabetes, Blind, Deaf, One leg missing! This type of information all helps when we come to X-ray the patient.
Please let me know if you would like another specific radiology user group meeting? We could use these subjects in future user group meetings for radiology users.
I look forward to some discussions on this subject over the coming weeks.

Hi,

Sorry - a long time since I've accessed this forum. We are deploying ICE at UHBristol but find the messaging functionality between ICE and CRIS is falling far short of that expected. Our top three wishes are:

1. Improve order comms message functionality and response as per HSS CRIS interface document RIS_PD154 OCS_Link_v1.8

2. Work on creating a link within radiology reports to allow web PACS to open at the correct patient images

3. There ave been radiation instances with users requesting on the incorrect patient - our Regional Radiation Proctection Advisor would like a confirmatory pop up screen to appear at the end of the request to be acknowledged by the user before sending to CRIS.

Hi Paul

It would be very useful if ICE could flag up an abnormal result perhaps by scanning for recognised text at the end of the radiologists report such as 'GOLD STAR'

When vetting requests the Radiologists would like to send a message to the referring clinician that thier request has been rejected and the reason why.

The system already uses the words within the report Text, using the patient alert function.
As you say we have set up two major alerts looking for the words
Gold Star - GOLD STAR chest X-ray report :- Unexpected chest cancer found - Cancer pathway initiated.
Red Star - RED STAR. This report contains URGENT FINDINGS - clinical evaluation required.
I too would like to see the second part of your request initiated, in that ANY RIS sends back the full reason for the cancelation or patient DNA.
I really think this is the type of question we need to put to Sunquest when we all meet on the 24th next month

Hi,

I am looking at Radiology reporting and eventually requesting through ICE and wondered what would happen if the GP requested one examination (say right foot) and the Radiographer decided it should be right ankle instead. In HSS CRIS we can change the event to the correct examination, but when we do this for CWS it means the report will not send back electronically (the system cannot match the event with the request as it now has a different exam code). Also, what would happen if we did two examinations on one request (eg request for right foot, x-ray right fot and ankle). Would both reports go back, or would they be seen as duplicates, as both relate to one request?

Has anyone any experience of these scenarios?

We have had some success with enhancing the HL7 message flows between ICE and CRIS. With regard to your issue of changing the laterality or exam code, this requires CRIS to send back XO (changed order) messages. For our LSP common CRIS instance we needed a service request to 'open the interface' as CRIS wasn't actually sending these messages although the HSS documentation said is should.

We are not yet in Alpine but the version of ICE we have (April 09.1) is sufficiently advanced that it can respond to the XO messages and change the request in ICE. We still have an ongoing issue with cancellations within a multi-order request.

You can also customise a translation table in CRIS to send out more variety of messages so that we can show accepted, rejected requests, requests on a waiting list, planned or appointments. The statuses are mapped to custom status codes in ICE for display. Setting up the table is under your control in CRIS (with some training and advice from HSS) but to activate it requires a service request.
